How Our Health Action Plans Work
Our new, streamlined, simplified, and flexible health action plan program puts you in the driver’s seat and allows us to help you create a highly personalized plan to meet your individual health goals and tackle your personal obstacles. One size doesn’t fit all when it comes to change readiness, but every step towards better health counts, so we’ll meet you where you are and work at your pace.
Our Nourish, Move, Thrive, and Recharge health action plans include:
- Intake assessment form to evaluate your current health and habits and help you determine the best areas to focus upon.
- Initial appointment appointment with a health coach to define your goals and customize an action plan that includes available onsite and virtual resources and services. (You’ll receive your $100 HAP credit after your initial appointment.)
The Healthy Habits Health Action Plan is a video-based program that doesn’t require an intake or health coach appointment for completion.
Our Track for Success Virgin Pulse quarterly challenges require participants to set a goal and track their habits in Virgin Pulse.
