What you need to know
This Health Action Plan makes you more aware of the reason why you make certain food choices and how the pattern of these choices can affect your overall health. Participating in the Food Connection plan will help you learn how to be a more mindful eater.
How the Plan Works
This is a 12-week program that helps you be more mindful about the food choices you are making.
After completing a questionnaire and three Food Connection classes (either in person or via Skype), a health coach will meet with you to review your assessment and answer any questions you may have.
Employee spouses are not eligible to participate in this Health Action Plan. But, those enrolled in Sandia medical coverage can complete certain activities for up to $50 each year. Go to Join a Health Action Plan to learn more.
Your Checklist
Your first step is to schedule a Food Connection appointment with an Employee Health Services health coach and complete a pre-assessment questionnaire.
Attend three Food Connection classes. You can attend these in person or via Skype.
If you are unable to attend the classes, schedule one-on-one sessions with your health coach.
After you complete the Food Connection class series, schedule a final appointment with your health coach.
You will receive a post-assessment in your email in the 11th week of the program. Bring your completed post-assessment to your final appointment.
