What you need to know
This Health Action Plan makes you more aware of the reason why you make certain food choices and how the pattern of these choices can affect your overall health. Participating in the Connecting with Food plan will help you learn how to be a more mindful eater and how to make better food choices for overall health.
How the Plan Works
This 12-week program helps you be more mindful about the food choices you are making.
After completing a pre-assessment questionnaire you will attend a one-on-one session with a health coach to review your assessment and answer your questions. Attend the four-class series via Teams to understand the details of your assessment score and ways to improve your mindfulness, food quality, and overall connection to food.
Employee spouses are not eligible to participate in this Health Action Plan. But, those enrolled in Sandia medical coverage can complete certain activities for up to $100 each year. Go to Join a Health Action Plan to learn more.
Your Checklist
Your first step is to complete a pre-assessment questionnaire and attend a one-on-one initial consultation with a health coach.
Attend four Connecting with Food classes.
If you are unable to attend all the classes, contact your health coach to review alternative class dates or other options.
After you finish the class series, complete your post assessment and submit to your health coach. You may also schedule another one-on-one session if you would like.
