What you need to know
Getting a good night’s sleep is just as important to your wellbeing as a healthy diet and exercise.
This Health Action Plan is for you if you find it difficult to fall asleep, sleep through the night without waking, or if you still feel tired after a good night’s sleep.
How the Plan Works
You will complete a pre-assessment about your sleep patterns and meet with a health coach to review your responses and determine what lifestyle changes you may need to make to improve your sleep.
After 90 days, you and your coach will meet to measure your progress and discuss additional resources or referrals that may be needed.
Employee spouses are not eligible to participate in this Health Action Plan. But, those enrolled in Sandia medical coverage can complete certain activities for up to $50 each year. Go to Join a Health Action Plan to learn more.
Your Checklist
Your first step is to complete a questionnaire and schedule a 60-minute initial sleep appointment. At your first appointment, you will review your responses and determine what changes you want to make to help improve your sleep.
After 90 days, you will complete a post-assessment questionnaire and attend a 60-minute exit appointment to review your progress. Your coach can provide additional resources, as needed.
