What you need know
Allergies and/or asthma getting you down? Learn about self-care, medications, natural remedies and/or immunotherapy you can use to decrease both allergy and asthma symptoms and improve your quality of life.
This Health Action Plan is offered during typical allergy seasons: February through May and August through October.
How the Plan Works
Attend a group clinic or initial appointment to learn more about your condition and schedule follow-up appointments. Asthma sufferers can also attend an additional Asthma Group Clinic. After 90 days, you will complete and return a post-assessment questionnaire.
Employee spouses are not eligible to participate in this Health Action Plan. But, those enrolled in Sandia medical coverage can complete certain activities for up to $50 each year. Go to Join a Health Action Plan to learn more.
Your Checklist
Your first step is to complete the pre-assessment questionnaire and bring it to your first group clinic or appointment.
Attend an Allergy Group Clinic (NM participants) or initial appointment (CA participants) to:
- Learn about self-care and medication use, including over-the-counter treatment options
- Review individual allergy health history
- If appropriate, receive prescription medications for allergy treatment
- Schedule follow-up appointments, which may include a referral to onsite or community providers
If you have asthma, also attend the Asthma Group Clinic (NM only) to:
- Learn environmental controls symptoms management
- Reduce modifiable risk factors
- Discuss medication self-treatment and inhaler skills
- Discover holistic approaches and natural remedies
After 90 days, complete and return your post-assessment questionnaire.
