What you need know
Allergies and/or asthma getting you down? Learn about self-care, medications, natural remedies and/or immunotherapy you can use to decrease both allergy and asthma symptoms and improve your quality of life.
This Health Action Plan is offered during typical allergy seasons: February through May and August through October.
How the Plan Works
View educational allergy and/or asthma videos to learn more about your condition and schedule follow-up appointments as necessary. After 90 days, you will complete and return a post-assessment questionnaire.
Employee spouses are not eligible to participate in this Health Action Plan. But, those enrolled in Sandia medical coverage can complete certain activities for up to $100 each year. Go to Join a Health Action Plan to learn more.
Your Checklist
Your first step is to complete the pre-assessment questionnaire(s). We will contact you to submit your pre-assessments.
View Attack Allergies Before They Attack You to learn about self-care and medication use, including over-the-counter treatment options. If your symptoms persist after implementing the environmental and self-care recommendations, meet with our onsite pharmacist to review self-care and medication treatment options.
If you have asthma, also view Asthma: Symptoms, Risk Factors, Treatments to learn environmental controls symptoms management and reduce modifiable risk factors. If your symptoms persist after implementing the environmental and self-care recommendations, meet with a Health Management Clinic provider to review self-care and medication treatment options.
Complete your post assessment questionnaire(s) and return to your clinic provider.
