Coordinating Care with Your Provider
Your primary care provider (PCP) is the best place to start for your health care. Over the long term, a PCP who knows your health history, your habits and your personality can more easily recognize signs that indicate a change in your health.
Sandia’s clinic professionals understand the importance of this relationship, and when you come to us for an emergency or another service, we can easily coordinate your care with your PCP before or after an on-site clinic visit.
To share your health information, you need to complete an authorization to release medical information that the clinic will keep on file. This allows the clinic to share notes and test results from your visits with your PCP, and vice versa.
To get a copy of the authorization form, contact HR Solutions. After you complete it, fax it to our secure fax line at 505-845-8190.
Accreditation
Sandia clinics are accredited through the Accreditation Association for Ambulatory Health Care.
They are also NCOA-recognized Patient Centered, Community Connected Care facilities.
Complete your health assessment
Completing an annual health assessment enables you to evaluate your current health status, track progress and identify any potential setbacks. By pairing the health assessment with health coaching, you create a powerful strategy for heightened awareness and actionable steps that lead to improved well-being.
