Health Action Plans
To improve accessibility and guide you on your Wellness Journey, Health Action Plans are now 100% video-based within the Wellworks For You digital platform, which can be accessed via your desktop or application on your device.
To access the Health Action Plans, employees and spouses should sign into the Wellworks For You, bookmark on your desktop and download the application on your device and watch the Navigation Video to best utilize the platform, including how to find the Health Action Plans. Watching the Navigation Video will earn you $100 in your Sandia sponsored Health Reimbursement Account (HRA) or Health Savings Account (HSA) for the following year.
Both employees and spouses have access to the same Health Action Plans. If you are enrolled in a Sandia medical plan, after completing a Health Action Plan in its entirety, including the pre- and post-surveys, you will receive $300 for your HRA or HSA. Employees and spouses must complete all activities before Dec. 1 to earn funding for the next calendar year.
How to choose your Health Action Plan
- Login to your Wellworks For You account and go to the Learning Center
- Within the Learning Center, find the “Health Action Plans (HAP) Options” section and select “view all.”
- Choose the HAP in which you would like to take part. Once you complete the entire program, you will be awarded $300 in your HRA/HSA for the next calendar year
- Remember, you only get credit ($300) for completing one Health Action Plan annually, but you can complete as many programs as you wish.

Complete your health assessment
Completing an annual health assessment enables you to evaluate your current health status, track progress and identify any potential setbacks. By pairing the health assessment with health coaching, you create a powerful strategy for heightened awareness and actionable steps that lead to improved well-being.
